When the Tip Is Pulled Up (Contracture)

A contracted nose has many causes. If inflammation was left untreated after a previous nose surgery, if the tissue contracted because the body recognized the implant as a foreign object, or if the tip cartilage was not properly released and secured, contracture can occur and the tip can turn up or become deformed.

This is the most difficult type of revision, and if done incorrectly, there is a high chance of needing another revision within a few years.

Surgery requires a surgeon who understands the structures of the nasal tip to fully release the adherent tissue, adequately mobilize and secure the autologous tissue at the tip, and lower the tip to a normal height and position.

Q.What kind of clinic should I choose for contracted nose revision?
A.

A contracted nose develops when inflammation after a previous surgery was not treated, when the tissue contracted because the body recognized the implant as a foreign object, or when the tip cartilage was not properly released and secured. In revision surgery, a surgeon who understands the structure of the nasal tip fully releases the adhered tissue, then releases and secures the autologous tissue at the tip to bring it down to a normal height and position. So check whether the cause is diagnosed first and whether the surgeon explains which cartilage will be used.

Q.My nasal tip is hard and turned up. Can it be brought down?
A.

There are very few cases in which an upturned nose cannot be corrected to a reasonable degree. An examination can determine how far the tip can be lowered. Our cartilage repositioning + tip support technique can lower the upturned tip while also making it feel soft and supple.

Q.I've had nasal tip surgery once, but the tip became too hard. Will I need rib cartilage for revision surgery?
A.

We avoid using rib cartilage whenever possible. Tip revision can be performed with small amounts of ear cartilage and septal cartilage, so we reuse previously used cartilage or perform surgery with only a small amount of cartilage.

Q.Why does a septal extension graft make the nasal tip hard and immobile?
A.

The septal extension graft, the most widely used method for raising the tip, firmly connects the rigid septal cartilage, which acts as a pillar between the nostrils, to the soft alar cartilage that controls tip movement, so the tip does not drop.

As a result, the tip has difficulty moving up and down independently and becomes hard.

That is why Sehyun's tip support technique separates the cartilage and uses minimal fixation to preserve movement of the nasal tip.

Q.What are the signs of nasal contracture?
A.

After surgery, the tip may gradually turn up, the nose may look shorter, or it may feel firmer or become more asymmetrical. Symptoms and how they progress vary from person to person, and contracture cannot be diagnosed solely because the tip looks upturned. If these changes continue, the condition of the tissue and the cause should be checked through an examination.

Q.How is contracted nose revision performed?
A.

After examining the cause of the contracture and the extent of tissue adhesions, we plan to release the contracted tissue and adjust the position of the tip cartilage and its support structure. When replacing the implant alone cannot solve the problem, the surrounding tissue must also be addressed.

The surgical method depends on the remaining autologous tissue and skin condition, and results and recovery vary from person to person.
