It is true that Sehyun's cartilage repositioning raises the nasal tip vertically and makes its shape
slimmer, but a bulbous nose can have many causes,
so correcting it properly requires a customized plan that addresses the cause.

East Asian noses typically have thick skin and weak cartilage.
So if only the alar cartilage is refined, it is covered by thick subcutaneous fat and skin,
and you may hardly notice that the tip has become slimmer.
That is why correcting a bulbous nose requires a surgical plan that addresses both subcutaneous fat and cartilage.

CASE 1
With Sehyun's cartilage repositioning + tip support technique, the implant that raises the bridge is placed only as far as the sturdy septum extends, and autologous cartilage is used to pull the tip cartilage forward and reposition it.
Unlike conventional methods, in which adding cartilage to the tip thickens the columella, Dr. Yoo Hyun-seok's method uses minimal cartilage with little bulk, slimming the tip while keeping it flexible.
CASE 2
Part of the excess subcutaneous fat layer of the tip is removed, then the alar cartilage is corrected.
※ If the skin of the nasal tip is thick, part of the dermis can be reduced, but removing too much dermis can damage the capillary network, reducing blood circulation and potentially causing skin necrosis. For this reason, the dermis must not be overly reduced, and removal focuses on the subcutaneous tissue.
CASE 3
Part of the alar cartilage is trimmed to reduce bulk in the tip and shift the tip's position.
However, because many East Asian noses have thick skin and a thick subcutaneous fat layer, trimming cartilage alone often does not reduce bulk much, so the soft tissue is also addressed as needed.
CASE 4
If the alar cartilage is excessively wide, part of it is removed before the splayed alar cartilages are drawn together.
If the subcutaneous fat and dermis are thick, the soft tissue is corrected as well.
Also, excessive fixation when suturing to the septum can make the nasal tip stiff.

A bulbous nose has several possible causes, such as wide or splayed alar cartilages, a low columella with flared nostrils, or a thick layer of subcutaneous fat at the tip, so it matters whether the correction is planned to match the cause. Asian noses tend to have thicker skin and weaker cartilage, so correcting only the alar cartilages may make little visible difference.
At Sehyun Plastic Surgery, Chief Surgeon Dr. Yoo Hyun-seok, a board-certified plastic surgeon, personally assesses the soft tissue and alar cartilages of the tip, then plans and performs cartilage repositioning together with correction of the subcutaneous fat. Bulbous nose surgery takes about 1 hour under sedation and local anesthesia, and daily activities can be resumed after 2–4 days (recovery varies from person to person).
The surgical plan differs depending on whether the subcutaneous fat layer of the tip is thick, the alar cartilages are large or spread apart, or the column supporting the tip lacks sufficient height. When both the fat layer and cartilage contribute to the bulbous shape, a method that corrects both tissues is considered.
The alar cartilage of the tip is repositioned at the planned height and orientation, then secured again. The position of the cartilage is chosen with the angle and length of the tip in mind.
A support structure is planned to match how much the tip is raised. The tip support technique divides cartilage into several pieces to support the tip, taking both tip support and tip movement into account.
Removing too much dermis can damage capillaries, leading to reduced blood circulation or skin necrosis. Skin and subcutaneous fat thickness vary from person to person, so the extent of removal must be adjusted, and we plan to reduce only as much as needed, focusing on the subcutaneous tissue.
