What Is Revision Rhinoplasty?

Revision rhinoplasty is surgery performed again when you are unhappy with the shape after a previous nose surgery or when side effects have developed.
The reasons people consider another surgery fall into two main groups.

The first is when you are unhappy with the shape. This includes when the two sides don't match or the nose feels crooked,
when it is too high and looks artificial or, conversely, shows almost no improvement, and when the nostril wings look wide.

The second is side effects that need treatment: when the implant shows through or pushes out through the skin,
when the nose turns red with lasting pain or discharges pus, and when contracture pulls the tip up and shortens the nose.
In these cases, treating the cause takes priority over improving the nose's appearance.

Some people believe rib cartilage must be used to support the tip
for a 3rd or subsequent revision rhinoplasty. Some clinics do advise this.
If ear cartilage is insufficient, rib cartilage may even be recommended for a 2nd revision.

However, rib cartilage surgery is more involved, patients recover more slowly, and it can cause a stiff tip, scarring
and other side effects, so it is best kept as a last resort whenever possible.

TIMING

When Is the Right Time for Revision Rhinoplasty?

If you are having surgery again because you are unhappy with the shape, we recommend waiting about 6 months after the first surgery, until the tissue beneath the skin has recovered to some degree.
Over those 6 months, the shape can improve as residual swelling goes down and the nose settles, and the tissue also needs sufficient time to recover.

If there are side effects, they are treated first, and the revision is done after the tissue has recovered.
If it is better to take out the existing implant during treatment, the implant is removed, and the nose is reshaped after a certain period.

Many people worry that there won't be enough cartilage for a second or third revision.
Sehyun uses the tip support technique, which does not add large cartilage grafts over the whole tip but secures only the points that must be fixed, one by one, so less cartilage is needed.
We also consider using the remaining cartilage or reshaping cartilage that has already been used.

Characteristics of Materials Used in Revision Rhinoplasty

The surgical process and its difficulty depend on which materials are used.

Silicone Implants

Mainly used to build the nasal bridge. They have the advantage of making a consistent shape relatively easy to achieve, but depending on the individual, the implant may show through the skin or cause a foreign-body reaction.

Ear cartilage

Used to create a soft nasal tip. If the ear cartilage used in a previous surgery is still in good condition, it is reused to reduce the amount of new cartilage needed.

Septal cartilage

The cartilage that forms the partition inside the nose, used to build the support structure of the tip.

Autologous Rib Cartilage

Cartilage harvested from your own ribs. A relatively ample amount can be harvested, and its strength and elasticity make it suitable for areas that must bear load, such as the tip and the columella.

Because harvesting the cartilage adds to the procedure, the operation can take longer, and the nature of the cartilage means slight warping may occur, so Sehyun uses it as a last resort whenever possible.

Autologous Tissue (Implant-Free)

If you have had problems caused by an implant, rebuilding the structure with your own cartilage or dermis may be considered. However, an implant may still be needed depending on the bridge height and structure, so the choice of material depends on each person's condition.
